Public Forms
Share a link that lets new patients start from one of your forms, before they log in.
A public form link lets a new patient start from one of your forms. They do not need to log in or make an account first. You share the link. The patient fills in the form. The platform then creates or finds their account.
You set this up on the Forms page. You need to be a Client Admin.
Before you start
Have these ready:
The form you want to share, already built on the Forms page
The place you will paste the link (your ad, landing page, or referral material)
Who does what: You do this yourself. You need to be a Client Admin. Your provider network (the licensed doctors who review orders and prescribe) does not touch it.
Time: About 5 minutes.
When to use a public form link
- A marketing campaign where you want people to fill in a form first.
- A referral or partner flow where patients arrive without an account.
Create and share a public link
Open the Forms page
In the left menu, click Forms. You see two tabs: Patient Intakes and Patient Consents. Open the tab that holds your form.
Turn on the public link
Find your form in the list. On its row, click Enable Public Link. A short message says Public link enabled.
Copy the link
On the same row, click Copy Public Link. A short message says Public link copied. The link is now on your clipboard.
Test it in a private window
Open a private or incognito browser window. Paste the link and press Enter. You should see your form's start screen, the way a new patient will. You can also click Open Public Link on the row.
Share it
Paste the link into your ad, landing page, or referral material. Anyone who clicks it starts from your form.
Turn it off when you are done
When the campaign ends, go back to the row and click Disable Public Link. A short message says Public link disabled. The old link stops working.
Remember — a public link keeps working until you turn it off. If you change your intake form or end a campaign, disable the old link so patients stop arriving through it.
How patient matching works
This is the part people worry about most. The platform matches each form submission by email address.
- New email: the platform creates a new patient account.
- Email already in your system: the submission attaches to that same patient. No duplicate account is made.
The match uses the email the patient types into the form. Make sure your form asks for an email address, so each submission reaches the right account.
Good habit — use each public link for one campaign only. Give it an end date. When the campaign ends, turn the link off.

Common questions
Will two links from the same person make duplicate accounts?
No. The platform matches by email.
- Same email as before: the answers attach to the existing patient.
- A brand new email: a new patient account is created.
What does the patient see when they open the link?
They see your form's start screen, then they fill it in.
- They do not need to log in first.
- After the form, they move into the normal account flow.
- Test it in a private window to see exactly what they will see.
Can my staff turn on public links?
No. Public links are a Client Admin task.
- Staff can open the Forms page, but cannot enable or disable public links.
- When staff need an existing patient to finish a form, they request it from the patient's profile. See Unblock Patients.
Do public links stop working on their own?
No. A public link stays live until you disable it.
- Old links keep collecting sign-ups even after a campaign ends.
- Turn a link off with Disable Public Link when you no longer want it.
How do I track two campaigns separately?
Give each campaign its own form, then its own link.
- On the Forms page, use Duplicate to make a copy of the form.
- Turn on a public link on each copy.
- Now you know which link each patient came through.
